ESCAPE: "Effects of long-term exposure to air pollution on health in European cohorts"(2008-2012)

Study objective:
The study investigates effects of long-term exposure to air pollution in Europe on human health. The background is that current estimates of the European health impact of especially fine particles in the air are large. However, these are primarily based on exposure response relationships establised in studies from North America. There is an urgent need to perform studies in Europe on recent and current exposures, and to use refined exposure assessment tools.

Specific aims:

  • to develop a flexible methodology for assessment of population exposure to air pollution focused primarily on fine particles, particle composition, and nitrogen oxides
  • to apply this exposure assessment methodology on existing cohort studies of mortality and chronic disease in Europe that have been selected based on their potential to quantify relationships between exposure and health response
  • specifically, to investigate exposure-response relationships for (a) adverse perinatal health outcomes, and development of diseases such as asthma in children;  (b) intermediate respiratory disease endpoints in adults; (c) intermediate cardiovascular disease endpoints in adults; (d) all-cause and cause-specific mortality, and cancer incidence
  • to develop a data base for quantitative estimates of the health impacts of air pollution for all of these health endpoints for the European population
  • to develop recommendations for improvement of the design and implementation of future studies on health effects of long-term exposure to air pollution in Europe.


Specific aims of work package 5 “cardiovascular disease endpoints” (coordinated by EPI II) are:

  • To quantify the impact of ambient air pollution on markers of inflammation, and gene-environment interaction
  • To quantify the impact of ambient air pollution on the risk for build-up of preclinical atherosclerosis measured by intima-media thickness (IMT) of the carotid artery
  • To quantify the impact of ambient air pollution on blood pressure and prevalence of hypertension
  • To quantify the impact of ambient air pollution on incident coronary events

Study design:
The ESCAPE project is organized as a set of seven, interlinked work packages (WPs). The Institute of Epidemiology II (EPI II) takes part in WP2 (exposure assessment) and WP6 (cancer incidence and mortality), and coordinates WP5 (cardiovascular disease endpoints).

Study population:
Several cohort studies spread over Europe. Helmholtz Zentrum München brings in to ESCAPE the following cohorts: KORA (EPI II) and ECRHS Erfurt (Institute of Epidemiology I) as adult cohorts and the Munich birth cohorts GINI and LISA (both EPI I).

Study area:
Most European countries participate in ESCAPE, thereof cities and regions from Austria, Belgium, Finland, France, Germany, Greece, Hungary, Italy, Lithuania, the Netherlands, Norway, Poland, Spain, Sweden, Switzerland, and the UK.
